Cadence

Monthly · set by statute
“Fees are payable to and must be submitted to the commissioner monthly before the 25th of each month following the month of collection”
Minn. Stat. §403.11 subd. 1(d)

Quarterly below $250.00 of monthly amount due. The product never moves your cadence on its own: it shows the state’s own threshold against the computed amount, and the agency assigns.

“Fees are payable to and must be submitted to the commissioner monthly before the 25th of each month following the month of collection, except that fees may be submitted quarterly if less than $250 a month is due”
Minn. Stat. §403.11 subd. 1(d)

Annual below $25.00 of monthly amount due. The product never moves your cadence on its own: it shows the state’s own threshold against the computed amount, and the agency assigns.

“or annually if less than $25 a month is due”
Minn. Stat. §403.11 subd. 1(d)

The thresholds are permissions, not instructions. The engine shows the amount a period computed to beside the state’s own words and flags where a different cadence appears to be available; changing cadence is the provider’s decision and the department’s to accept.

Quarterly and annual filers use the two-page form, whose second page breaks the period down month by month — which is why the count field repeats per service month.

“Fees are due to the Minnesota 9-1-1 Program before the 25th of the month following the month(s) of collection [MN Statute 403.11, Subd.1(d)]. Fees remitted after the due date are subject to a collection penalty [MN Statute 16D.11].”
Minnesota Telephone Fees Remittance Form

The statute says “before the 25th”, so the engine schedules the 24th — the earlier and therefore the safer reading of the state’s own word. The verbatim wording is shown beside the date.

Minnesota publishes no weekend or holiday rule for this deadline, so the engine does not move the date.

Authority conflict

Two of the state's own documents disagree.

This is never settled in code. Both positions are reproduced as the state wrote them, the engine’s posture is stated plainly, and the period is flagged for review so a person decides before it closes.

The effective date printed on page one of the current form

Flagged for review

Page one of the current form states an effective date a year earlier than the rest of the document. The filename and the form’s own page-two rate-history table both say April 1, 2026.

What the engine does: The engine dates this window from April 1, 2026, following the filename and the form’s own history table. Anyone reading page one alone will see a different year, which is why the difference is recorded rather than quietly corrected.

What you can do: No action is expected. Mention it to the department at ECN.Remittance@state.mn.us if a filing is ever queried on the effective date.

Counting rule

Per customer access line or other basic access service, including trunk equivalents as designated by the Public Utilities Commission for access-charge purposes, and including wireless services. The fee must be the same for all customers. The statutory ceiling for the 911 fee alone is 95 cents a month per line; the amount actually in force is set below it and is recorded in the rate history.

Interconnected VoIP is in scope by the statute’s own terms rather than by inference: §403.11 names the “wireless, wire-line, or packet-based telecommunications service provider” throughout — in the fee assessment, in the collection duty and in the sworn-declaration subdivision.

Per-customer cap
No per-customer or per-location line cap appears in the statute.

“customer access line or other basic access service, including trunk equivalents as designated by the Public Utilities Commission for access charge purposes”
Minn. Stat. §403.11 subd. 1(a) and (c)
“may not be more than 95 cents a month for each customer access line or other basic access service”
Minn. Stat. §403.11 subd. 1(c)

Collection allowance

Minnesota grants no collection or administrative allowance. Both research passes read Minn. Stat. §403.11 in full, all six subdivisions, and found no provision letting a wireless, wire-line or packet-based provider retain any part of the fees collected. The full amount collected is remitted.

Rate history

4 windows on record

The engine resolves the window that covers the filing period, never the latest one. A back period therefore prices at the rate that was in force for it.

The form’s own rate-history table writes this window as “For reporting periods 7/01/2024 to 8/30/2024” and the window that follows as “For reporting periods 9/1/2024 to 6/30/2025”. Read together, the two sentences leave 31 August 2024 inside neither range. This window is therefore recorded as running through 2024-08-31, the day before the start date Minnesota itself publishes for the following window, so that no day of the state’s own history is left without a rate. The state’s published end date, 2024-08-30, is reproduced here rather than replaced quietly. Both sentences were re-read from the current form on 2026-09-03 and are unchanged.

A second pass on September 22, 2026 confirmed the 86-cent amount and its July 1, 2024 start from the department’s own 2024 form, and the Public Utilities Commission’s January 2025 notice dates the next surcharges from September 1, 2024.

Confirmed by composition on September 22, 2026. No single independent document states 99 cents from July 1, 2025 through March 31, 2026, but separate documents together do, with no conflict. The department’s July 2025 form gives the 99-cent combined amount and its July 1, 2025 start. The Department of Revenue lists 911 at 80 cents, TAM at 4 cents and 988 at 12 cents from July 1, 2025, the same components on this window. The Public Utilities Commission reduced the 3-cent TAP surcharge to zero in Docket 25-355 and issued a TAP reporting form without the surcharge for filings after April 1, 2026, which closes this window at March 31, 2026.

The combined total, $0.96, is numerically the same as the total the superseded three-fee form printed from July 1, 2025. The two are different windows on different forms, and an engine keyed on the total rather than on the window would collapse them.

Zero-report rule

The state requires a declaration you write and sign

The state's requirement is reproduced below, and that is where this product stops. It composes no declaration wording for you, for this state or for any other — the declaration is your own act.

Trigger
The duty is conditional, not periodic. It arises where the commissioner disputes the accuracy of a fee submission, or where no fees are submitted, and it becomes enforceable once the commissioner gives notice and 90 days pass. There is no standalone zero-report form, no checkbox, and no unconditional duty to file a declaration for a quiet month.

“If the commissioner disputes the accuracy of a fee submission or if no fees are submitted by a wireless, wire-line, or packet-based telecommunications service provider, the [provider] shall submit a sworn declaration signed by an officer of the company certifying, under penalty of perjury, that the information provided with the fee submission is true and correct. The sworn declaration must specifically describe and affirm that the 911 fee computation is complete and accurate.”
Minn. Stat. §403.11 subd. 1a
“fails to provide a sworn declaration within 90 days of notice by the commissioner that the fee submission is disputed, the commissioner may estimate the amount due … and refer that amount for collection under section 16D.04.”
Minn. Stat. §403.11 subd. 1a

The product surfaces this requirement and stops. A sworn declaration is the officer’s own act; the product does not draft one, does not template one, and does not suggest wording for one. Subdivision 1b separately authorizes the commissioner to require an independent examination of fees under attestation standards.

Record retention

not published

Record retention · not on record

Minnesota publishes no provider-side record-retention period for 911 fee remittance records. Subdivision 1b authorizes the commissioner to require a certified public accountant’s examination of fees “if necessary”, but specifies no period to keep records for.

Searched: Minn. Stat. ch. 403 and the department’s Emergency Communication Networks pages. Both research passes reached the same absence.

Registration

There is no dedicated 911-fee registration portal or form. The published channel for establishing or updating a remittance relationship is email, followed by filing the standard remittance form.

Separately, and not specific to the 911 fee, Minnesota requires telecommunications carriers and resellers to hold a Certificate of Authority from the Public Utilities Commission in order to operate in the state at all. That pointer is weakly sourced — it surfaced in a search summary rather than on a Commission page — and should be treated as a pointer, not a verified requirement.

The state’s own form

Form
The state publishes no form identifier. Minnesota publishes no numeric form identifier. The department identifies the form by title alone: “Minnesota Telephone Fees Remittance Form”. Two variants exist — a one-page version for monthly filers and a two-page version for quarterly and annual filers, the second adding a monthly breakdown page.

Everything else this record does not know

Counting rule · not on record

The trunk-to-line equivalency multiplier is not stated in §403.11. The statute delegates it to the Public Utilities Commission’s access-charge designations, and that methodology was not retrieved. A provider counting trunk equivalents should confirm the multiplier before filing.

Searched: Minn. Stat. §403.11 in full and the department’s fee pages.

Portal address · not on record

No dedicated online remittance or filing portal was found. The published vehicle is the form plus mail or ACH.

Searched: The department’s 911 program and service-provider fee pages.

Rate history · not on record

Fee amounts before July 1, 2024 are not published on the current form, which directs enquiries to the department. Windows before that date are therefore not carried, and a period before July 1, 2024 is refused rather than priced.

Searched: The current form’s rate-history table and the superseded revision.

Zero report · not on record

Whether “no fees are submitted” alone triggers the sworn-declaration duty, or whether the duty arises only once the commissioner gives notice, is not resolved by the statute’s wording. No department-published declaration template was located either.

Searched: Minn. Stat. §403.11 subd. 1a and the department’s published materials.

Notes carried on this record

The 988, TAM and TAP fees ride the same form, the same schedule and the same remit-to address as the 911 fee, collected as one combined per-line figure. They are recorded as fields on the state’s own 911 remittance form, never as separate filings.

The form prints a single combined “911 & 988” line of $0.92. If a 911-only figure is ever needed from the form itself, that split does not exist there — it comes from the department’s fee page.

The form carries a standing certification block signed on every submission. That block is the state’s own text and is the provider’s to sign; the product never composes attestation language.
